With the issue of accessibility to health and wellness resources in underprivileged and elderly communities already being so low, extremities have worsened since the pandemic. WISE Health SF focuses on working with organizations to design, implement, and evaluate solutions to major public health issues.

The model for WISE Health was created as a result of the overwhelming and constant observations of the need for more innovative, tailored, and relevant approaches to the health and wellness of vulnerable populations living throughout the Bay Area. WISE Health offers a variety of community health programming that can be tailored to meet the needs of all ages.

The problem is the low socioeconomic communities in the San Francisco, Bay Area lack accessibility to resources and opportunities to receive medical care and nutritional wellness due to insufficient funds, limited mobility, and lack of knowledge.

The purpose is to create a toolkit in which individuals who receive it will be aware of upcoming Healthy Street events that are being offered by WISE Health SF, supply health and nutritional information, and allow recipients to track their previous and upcoming healthcare visits.

Over 4 months a research study was conducted to effectively design a toolkit for WiseHealthSF and the Black Health Initiative that would meet the needs of the targeted audience. The research study addresses the problem, purpose, research methods, methods of procedure, solution analysis, and assessment. Due to the research being so vast only the design process of this project will be addressed, but the entire research report can be read here.
